Not strictly, since Google can find a small site by following internal links. It becomes valuable as soon as you have deep pages, a large number of URLs, or new content that should be indexed quickly, because it shortens discovery time considerably.
There is no official hard cap, but 50,000 URLs or 50 MB uncompressed is the standard limit for a single file. Larger sites should use a sitemap index that points to several smaller sitemap files instead.
Google largely ignores both fields, so set realistic values rather than over-engineering them. Leave priority flat across normal pages, and use lastmod accurately to signal real changes instead of refreshing it daily out of habit.
By hand is fine for a small static site, but anything that changes should be generated automatically each time content is published. A stale sitemap carrying wrong lastmod dates is actively worse than having no sitemap at all.
